The influence of white coat effect on the diagnosis of metabolic syndrome

Abstract
Background: The dichotomous criteria for blood pressure in making a diagnosis of metabolic syndrome (MS) might have potential measurement error. As Ambulatory blood pressure (ABP) can provide more accurate measurement, the aim of this study is to compare two rates of diagnosis of MS when using ABP or conventional blood pressure (CBP).

Methods: This study analyzed 129 persons from community based cohort who completed 24hour ABP measurement, questionnaire, and clinical examination. CBP was measured by 3 physicians with mercury sphygmomanometer. 24hour ABP using TM-2430 (A&D Co.) was recorded at 15 min intervals from 07:00 to 22:00 and at 30 min intervals from 22:00 to 07:00.

Results: The mean age was 59¡¾11 and 42.6% of participants were men. Correlations between CBP and daytime ABP were r=0.505 (p < 0.001) in systole and 0.518 (p < 0.001) in diastole. Kappa index between CBP and daytime ABP was 0.92. The difference of diagnosis rate for MS between CBP versus ABP was not significant (p by McNemar=0.063).

Conclusions: This result indicates that accurate CBP measurement would give proper information for diagnosis of MS.
